CCN’s multi-disciplinary case management program, referred for services through contracted agencies and tracked through a centralized data system. CCN receives a fee plus upside gain sharing from its Medicaid HMO customers. To date, CCN has enrolled over 10,000 members and has reduced emergency room visits and hospital bed days by 18 and 24 percent respectively (per 1,000 population).
