Assignment of a Patient Care Representative or Specialty Care Representative:
Upon enrollment into CCN’s pharmacy programs, every patient is assigned a Patient Care Representative (PCR) or Specialty Care Representative (SCR) to welcome them into the program and assist them with all of their prescription needs. The same PCR or SCR will remain as the patient’s primary point of contact throughout their time with CCN and contact them monthly when refills are due.

Patient Eligibility and Insurance Verification:
CCN performs patient eligibility checks and insurance verification for all new patients and maintains this procedure on an ongoing basis. This ensures timely processing and reimbursement of claims while optimizing the benefit provided by the insurer.

Assist with Enrollment into Patient Assistance Programs:
Due to the high costs associated with HIV and other specialty medications, CCN encourages the use of and assists in enrolling eligible patients into manufacturer Patient Assistance Programs and other related programs such as copay assistance. CCN identifies these patients and provides the necessary forms to the hospital or clinic or directly to the patient and assists with completing these documents.

Prior Authorizations:
Medications, especially HIV and specialty drugs, often require prior authorization from the insurer. Because this process can be timely for hospital or clinic staff and prescribers, CCN offers to submit for prior authorizations when the insurer allows.

Referral Resources:
Through monthly interaction with the patients, CCN’s Patient Care Representatives (PCR) and Specialty Care Representatives (SCR) are able to identify social needs that may be present and are able to offer the patient resource referrals. These referrals include but are not limited to food, clothing, utility assistance, and transportation.

Toll-Free 24/7 Access to Board Certified Pharmacists:
Access to CCN offers all patients toll-free 24/7 access to a board-certified pharmacist in addition to toll-free access to the pharmacy and PCR/SCR staff during normal operating hours. Normal hours of operation are Monday through Friday, 8:30 am to 5:00 pm EST.

Free Customized Delivery:
CCN provides free delivery, via U.S. Mail, UPS, FedEx, or courier service, to all patients. Shipping is customized to meet the needs and preferences of each patient and offers real-time tracking and re-routing capabilities.

Fee Schedule Management:
CCN assists the clinic in developing and managing self-pay patient fee schedules within government requirements. These schedules are updated annually.

Customized Reporting:
CCN understands the vast and varied needs of its clients and works with them to develop customized financial, operational and regulatory reports.
