Jeffrey S. Palmer
CHAIRMAN AND CEO
An entrepreneur and leader in the 340B community, Mr. Palmer founded Coordinated Care Network while he was Executive Director of a Federally Qualified Health Center serving urban Pittsburgh. He served as CCN’s president and CEO from inception until 2006 when he transitioned to chairman and CEO. Part of Mr. Palmer’s health care experience comes from his 12-year tenure at the University of Pittsburgh Medical Center (UPMC) where he held various senior management positions in the areas of Surgical Services, Anesthesiology & Critical Care Medicine, Business & Finance and Molecular Neurobiology & Genetics. In addition to CCN, Mr. Palmer co-founded a surgical software company and a nutraceutical company. He has also consulted nationally in the areas of pharmacy access, technology development, and managed care. In 2003, Mr. Palmer was elected to the Ashoka Fellowship for his innovative work creating ways to finance and deliver healthcare for uninsured and semi-insured populations in the United States.
Thomas J. Pollack
PRESIDENT AND CFO
A Certified Public Accountant and former Senior Auditor for Deloitte & Touche, Mr. Pollack has been with CCN since 2000. He is well-versed in the areas of specialty pharmacy, health care, risk management, and managed care. During his tenure at CCN, Mr. Pollack has been responsible for overseeing, building and maintaining all aspects of the company’s financial, operational and human resource activities. Prior to joining Coordinated Care Network, he was Chief Financial Officer of Brownsville General Hospital in Brownsville, PA. He also served as Chairman of the Finance Committee for the Mario Lemieux Foundation for three years.
Lori L. Hunt, RN
VICE PRESIDENT, CLINICAL SERVICES
As Clinical Director since 2006, Mrs. Hunt has been instrumental in developing the customized HIV/Specialty Pharmacy Program at CCN. Responsibilities include senior management of Client Relations, HIV/Specialty Pharmacy Program, as well as CCN Case Management Services. Prior to joining Coordinated Care Network in 2000, she was Program Coordinator and Interim Executive Director for the William & Mildred Orr Compassionate Care Center, a nonprofit respite care facility in Allegheny County. Mrs. Hunt also served as the Director of Nursing for Fox Run Health System, a children and adolescent behavioral health organization in Ohio; and is currently an Officer on the Board of LivingMother.
Fredrick J. Grasser, MBA
VICE PRESIDENT, INFORMATION TECHNOLOGY & BUSINESS STRATEGY
Mr. Grasser is responsible for 340B operations, information technology, and business strategy. He has extensive experience in pharmacy management and the 340B program, employing analytics, data management and financial strategies in the development of CCN’s models. A graduate of the Masters of Business Administration program at Duquesne University, Mr. Grasser has also served as a member of Duquesne University’s Information Systems Management Advisory Board. Prior to joining the Coordinated Care Network staff in 2001, he worked as a software engineer at Management Science Associates.
David J. Giannangeli
VICE PRESIDENT, BUSINESS DEVELOPMENT & CORPORATE RELATIONS
During his tenure at CCN, Mr. Giannangeli has been instrumental in the company’s growth through new client sales, product and business development, project management of on-site pharmacy implementations and government relations. Prior to joining Coordinated Care Network in June 2005, Mr. Giannangeli worked for All-Pak, Inc in Pittsburgh as a National Accounts Manager responsible for Fortune 500 chemical and pharmaceutical clients. His experience included working closely with clients to operate within the regulatory requirements of the Federal Aviation Administration and the Department of Transportation, as well as major transport carriers such as FedEx and UPS.
VICE PRESIDENT, SALES & MARKETING
Mr. Montefiori brings nearly 30 years of sales, sales training and account management experience to CCN, much of that time in the pharmaceutical industry with Bristol-Myers Squibb and Novo Nordisk. He has extensive understanding of the HIV and Hepatitis markets having worked with providers in these therapeutic areas for the last ten years. He has a demonstrated track record of success winning the top sales award five consecutive years along with multiple other honors throughout his career.
Jeffrey J. Meis, RPh
VICE PRESIDENT, PHARMACY SERVICES
As CCN’s Vice President, Pharmacy Services, Mr. Meis directly manages all HIV/Specialty Pharmacy operations. A Pharmacist for over twenty years, Mr. Meis is a 1994 graduate of the University of Pittsburgh. Prior to joining the CCN staff in 2006, he was a retail pharmacist at Giant Eagle for ten years where he had many opportunities to work with the latest in pharmacy software systems. He also gained four years of management experience in a “top -10” volume store that prided itself on outstanding customer service and efficiency.
Faith M. Sedlak, Pharm D, AAhiVp
director of client & patient services
A 2006 graduate of Duquesne University, Dr. Sedlak started her career as a retail pharmacist at Giant Eagle and quickly moved into a position administering clinical programs. She joined CCN in 2012 to grow and develop the specialty customer service team using a patient centric approach. As Director of Client & Patient Services, Dr. Sedlak oversees the care teams made up of American Academy of HIV Medicine (AAHIVM) certified clinical pharmacists and Specialty Care Representatives working with patients and providers, ensuring the best-in-class customer service.
ANDREW “BERNIE” HEUER, RPH
DIRECTOR OF REMOTE OPERATIONS
As CCN’s Director of Remote Operations, Mr. Heuer, is responsible for all remote site pharmacy operations and ensuring they meet standards of accreditation, performance, and organization compliance . Mr. Heuer graduated from Duquesne University and started at CCN in January 2019. He has an extensive retail background with twenty plus years of multi-unit experience with responsibilities at the district management and regional VP levels with Rite Aid and district management at CVS and Kmart.
Candice B. Carr, PHR
HUMAN RESOURCES DIRECTOR
As a certified Professional in Human Resources (PHR), Ms. Carr joined the CCN team in 2012, bringing previous experience in Human Resources from both corporate and non-profit organizations. In prior roles as Operations Analyst, Human Resource Generalist, and Professional Services Recruiter, Ms. Carr worked in diverse areas of all HR disciplines such as Staffing Management, Compensation and Benefits, Employee Development and Labor Relations. She attended West Virginia University where she received her Bachelor’s Degree in Business Management with a focus in Human Resources and maintains her PHR (Professional in Human Resources) certification since 2010 and SHRM-CP (Society for Human Resources Management Certified Professional) since 2015.
